Stopping Throat Clearing

Instructions

A method of breaking the cycle of coughing/throat clearing → tingly/sore/tight sensations → coughing/throat clearing → etc. → etc. is presented in terms of three staged actions.

All three steps should be introduced, demonstrated and practiced in turn, so that each client understands how to implement each step should this be necessary.

Each client should be encouraged to try action 1 (sip of water) before trying action 2 (firm swallow). If neither of these two actions has the desired effect and the client feels that he or she must cough, then they should be encouraged to use action 3 (a dry cough). The three actions are explained below.

Technique

1. Sip of water:

  • Do not cough afterwards!

2. Firm swallow – for this, encourage clients to:

  • Take a sip of cold water and hold it in the mouth.
  • Place the flat of one hand against the upper sternum.
  • Drop the chin slightly (without pressing it too far towards or onto the chest).
  • When ready, swallow firmly, as if gently pushing down a large pea or a grape all in one go.
  • At the same time push down gently with the flat of the hand against the upper sternum.
  • Do not cough!
  • Take another sip of water if necessary.

3. Dry cough – for this, encourage clients to:

  • Place the flat of one hand against the upper sternum.
  • Take a deep breath.
  • Then, when ready, expel the air vigorously, making the sound /h/.
  • At the same time push down gently with the flat of the hand against the upper sternum.
  • Don’t cough!
  • Then take another sip of water (this sort of safe cough will likely dry the throat).
